Engaging primary care practices in rural N.C.: Lessons from the Heart Health Now study

With little informatics support, Heart Health Now engaged 219 small primary care practices in N.C. to help implement new evidence-based strategies to reduce cardiovascular disease (CVD) risk.

Samuel Cykert will discuss key implementation lessons and study findings from this statewide project and highlight emerging topics in the field.
